4. Documents to be submitted when requesting the disclosure of personal information, etc.
a. If you request the disclosure of personal information or notification of the purpose of use, we will send you a disclosure request form. Please fill out the necessary information on that form and submit it by mail to our "Contact Center."
The notification of the purpose of disclosure and use will be sent to the address of the individual confirmed by each invoice and identification document via recorded delivery mail.
b. If you request correction, addition, deletion of personal information, suspension of use, erasure, or cessation of provision to third parties, we will send you a correction request form. Please fill in the necessary information on that form and apply by mail to our "Contact Center."
Corrections will be made to the extent that it can be confirmed that the personal information belongs to the individual based on each invoice and identification documents. The results of the corrections (including cases where corrections could not be made) will be communicated by sending a written notice via recorded delivery mail to the address of the individual confirmed through each invoice and identification documents.
Also, please be aware that if you delete your personal information, suspend its use, or stop providing it to third parties, you may not be able to receive the services you are currently using.
c. The submitted invoices and identification documents will not be returned. Each invoice will be properly managed by our company. The identification documents will be disposed of by our company after a certain period of time.
d. In addition, we may contact you or your representative by phone or other means to verify the contents of each invoice and identification documents.
5. Fees related to requests for disclosure of personal information and notification of the purpose of use.
Regarding the disclosure of personal information and notification of the purpose of use, a fee of 500 yen (excluding consumption tax) will be charged for each request. Please note that if we notify you by mail, we may charge you for the actual costs separately. When sending the disclosure request form by mail, please enclose stamps or a postal money order worth 540 yen.
6. Persons who can request the disclosure of personal information, etc.
Requests for the disclosure of personal information can only be made by the individual concerned (the person identified by the personal information subject to disclosure) and their representative (a person delegated by the individual concerned or a legal representative such as a guardian). Additionally, when requesting the disclosure of personal information, you will be required to submit documents to confirm that the requester is either the individual concerned or their representative. Please be aware that if there are any deficiencies in the submitted documents, we will not be able to respond to the request for the disclosure of personal information.
